Driver’s License or valid government ID.

Please bring your health insurance card.

Please provide a complete list of all medications including over the counter medications, vitamins and supplements you are currently taking. Please include dosage and frequency for each medication.

Bring recent imaging reports and imaging discs for upload into our system and review. If you have prior operative records related to your current issue, please bring those as well. Most imaging centers will provide you a disc with your images. Please bring this disc to your first visit.

Some insurance plans require that you pay a copayment at the time of the visit.
